In consideration of my student being allowed to participate in the field trip, I hereby assume all risks in connection with the field trip, and I further release Main Street Baptist Church and the staff, employees, and volunteers from all claims, judgments, and liability for any injury or damage that the youth may have due to participation in the field trip, including risk connected therewith foreseen and unforeseen. I fully understand what is involved in the field trip (as listed on the back of the form), and I acknowledge that I have the opportunity to call the Youth Pastor and ask him about the field trip.

Mission Statement: The mission of Abide Youth is to abide in the love of Jesus and His word to glorify The Father, bear fruit, and prove to be His disciples. (John 15:1-11) I will abstain from any and all tobacco products. I will abstain from all alcohol. I will abstain from any inappropriate or immoral behavior. I will abstain from any inappropriate or foul language. I will fully cooperate and follow directions from my leaders. I will purpose to spiritually resolve any interpersonal relationship issues while on the trip with other team members, following the Matthew 18:15-17 principle. I agree that I am under the spiritual and doctrinal authority of Main Street Baptist for the duration of the trip. I understand that if I persist in demonstrating beliefs inconsistent with those contained in the BAPTIST FAITH AND MESSAGE (as listed on the website), I will no longer be able to participate in this field trip and possibly further trips. If I violate this agreement, I understand that I will be responsible for any damages or behavior conducted at my own expense. (initial below)
